Oahu, HI – Woman Dies After Chain-Reaction Crash on H-3 Fwy

Oahu, HI (November 21, 2025) – On Thursday, November 20, a 29-year-old woman died following a four-vehicle, chain-reaction crash on the H-3 Freeway. 

According to the Honolulu Police Department, the collision occurred at approximately 5:36 p.m. in the westbound lanes near the Moanalua Freeway overpass. HPD’s Traffic Division reported that the woman was driving west on H-3 when her vehicle was struck from behind by a car driven by a 19-year-old man. The force of the impact pushed her vehicle forward into another car driven by a 58-year-old man, which then collided with a fourth vehicle driven by a 31-year-old man.

Honolulu Emergency Medical Services responded to the scene and treated the 29-year-old woman before transporting her to a nearby hospital in critical condition. Unfortunately, she was later pronounced dead. Police confirmed that she had not been wearing a seat belt at the time of the crash.

Two additional occupants involved in the collision were evaluated by paramedics at the scene but declined hospital transport. HPD stated that neither speed, drugs, nor alcohol appeared to be contributing factors. Several lanes of the H-3 westbound were temporarily closed during the evening commute as emergency crews worked the scene.
